The recent development in legislation inherent to the Pharmacy put in evidence the need to offer an adequate formation to the pharmacists, in particular on those topics in which the pharmacist in the Community Pharmacy could be a real help for practitioners and specialists.

Federfarma Piemonte, in concert with the Order of Pharmacists and the University of Turin, has identified a big void to fill in the management of public health, in particular in the primary prevention and in the compliance; it also provides the availability and the collaboration of 1600 pharmacies of Piedmont to perform, in collaboration with the Regional Health Service, these activities in a coordinated and uniform way.

The project is based, thus, on the new role of the pharmacist, who through the monitoring of the assumption of the medications, the correct health information and the professional management of the self-testing contributes at the primary prevention of the lifestyles related to the diabetes, and  supports the diabetic patient, by contributing with the increase of the early diagnosis of the unrecognized diabetes and of the adherence to the chronic therapy.

This could bring a reduction of the inappropriate hospitalizations and the access to the emergency room for the diabetic patients and thus a consequent saving for the Regional Health System. This, obviously, only if a guideline is agreed and validated by medical doctors.

In Europe it was demonstrated that a more widespread health surveillance on the citizens/patients promotes the phenomenon of de-hospitalization with a consequent benefit of the quality of life of citizens, a lower onset of comorbidity, and a reduction of the healthcare costs.

The principal actions of the project argued the most problematic chronic diseases: diabetes, dyslipidemia, heart failure, chronic obstructive pulmonary disease.

In 2013 and 2014 many formation courses along Piedmont territory were performed to better reach the more than 3500 pharmacists working in the Region.

Then, a study was planned in which different types of questionnaires were administered to subjects with the double aim of enrolling potential patients and monitoring the therapy.

Regarding diabetes, in two waves about 15,000 questionnaires, divided into the two arms of the study, were administered and the data were entered into a database. The same methodology is being applied to the other three diseases: dyslipidemia, heart failure, chronic obstructive pulmonary disease. 

Given the success of the project on diabetes in Piedmont, the same initiative has been started in 2016 in other two Italian Regions, Umbria and Puglia.

Preliminary results are encouraging to continue the study also in other fields.
