Description:

The increase of the hospital cost and the ageing of the population impose more and more the shift of the treatment of the chronic diseases from the hospital to the territorial medicine; so it is necessary to quickly identify the subjects with an increased risk for chronic illnesses or sick persons but not recognized in order to ensure that they are borne by the Health Service.

Among chronic diseases, diabetes is considered a social illness due to its high prevalence in the population.

In Italy, the prevalence of known diabetes is 4.9%, it increases with age and it is more common in people with low social status; the prevalence is steadily rising, with an estimated 50% increase over the next twenty years.

Early diagnosis in these "unconscious" subjects allows to start the therapy in terms of information, intervention on lifestyle and drugs, minimizing the development of complications. Good therapeutic compliance allows to reduce the risk of such outcomes and their impact and cost on the use of services.

The project includes two separate routes:

- people with known type 2 diabetes that are enrolled in a pharmacy, when they have a prescription for antidiabetic drugs, are evaluated for adherence to therapies and to guidelines for managing the disease;

- to adults with obvious overweight status and therefore at risk of developing diabetes, a questionnaire to measure the risk index is proposed; this is the Findrisc, an internationally validated questionnaire, whose score allows to discriminate low-risk subjects - to whom only life-style counseling will be proposed - from high-risk subjects who will be advised to measure glycaemia at pharmacy and therefore, according to the resulting values, a visit by the treating physician.

This model includes a first phase in which an appropriate training is provided for the use of the intervention tools to the pharmacists working in the three target territories (Piedmont to cover the whole territory not yet covered by the pilot, Umbria and Puglia to assess the transferability) and a second operational phase, consisting in interventions directed to the distinct target audience. In order to detect the data and to monitor the results over time for those who give consent to the intervention, an online information system has been developed.

The following aspects are evaluated:

A) the effectiveness of intervention in the Piedmontese context, in particular in terms of reducing inadequate hospitalizations and access to First Aid for diabetic patients, impact on health inequalities and on economy.

B) the transferability of the model to two different contexts (Umbria and Puglia)

C) the adaptability of the tested and validated diabete model to COPD in Piedmont territory.
